Safety Waiver:

RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT PLEASE READ CAREFULLY BEFORE SIGNING

In consideration of being permitted to participate in any way in events, programs, and activities conducted by JD Athletics, LLC (the “Company”), the undersigned, on behalf of themselves, their spouse, and their minor child(ren), hereby acknowledges and agrees as follows:

1. Assumption of Risk

   The undersigned acknowledges that participation in athletic activities involves inherent risks, including but not limited to the risk of bodily injury, illness, permanent disability, death, and property damage. The undersigned knowingly and voluntarily assumes all such risks, whether known or unknown, foreseeable or unforeseeable, including those arising from the negligence of the Company or any of the Released Parties (as defined below).

2. Responsibility for Minor Participant

   The undersigned accepts full responsibility for the supervision, care, and conduct of their minor child(ren) during participation in Company activities. The undersigned agrees to ensure compliance with all rules, regulations, and instructions. In the event the undersigned observes any condition or circumstance that may pose a risk to the minor participant, the undersigned shall immediately withdraw the minor from participation and notify a Company representative.

3. Release and Waiver of Liability

   To the fullest extent permitted by law, the undersigned, on behalf of themselves, their spouse, their minor child(ren), and their respective heirs, assigns, personal representatives, and next of kin, hereby releases, waives, discharges, and covenants not to sue JD Athletics, LLC, and its directors, officers, officials, agents, employees, volunteers, participants, sponsors, advertisers, and, where applicable, the owners and lessors of premises used for the activities (collectively, the “Released Parties”), from any and all claims, demands, losses, or liability for any injury, disability, death, or damage to person or property arising out of or related to participation in the Company’s activities, whether caused by the negligence of the Released Parties or otherwise.

4. Indemnification and Hold Harmless

    The undersigned agrees to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, damages, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to the participation of the undersigned or their minor child(ren) in Company activities, including claims arising from the negligence of the Released Parties, to the fullest extent permitted by law.

5. Acknowledgment and Voluntary Execution

   The undersigned acknowledges that they have carefully read this Agreement, fully understand its terms, and understand that they are waiving substantial legal rights, including the right to bring a lawsuit. The undersigned further acknowledges that this Agreement is executed voluntarily and without any inducement or assurance of any nature.

Media Release Form:

MEDIA RELEASE AND AUTHORIZATION AGREEMENT PLEASE READ CAREFULLY BEFORE SIGNING

In consideration of participation in events, programs, and activities conducted by JD Athletics, LLC (the “Company”), the undersigned, on behalf of themselves, their spouse, and/or their minor child(ren), hereby acknowledges and agrees as follows:

1. Grant of Rights

   The undersigned hereby irrevocably grants to the Company the unrestricted right and permission to photograph, record, and/or capture the name, image, likeness, voice, and appearance of the undersigned, their spouse, and/or their minor child(ren) (collectively, the “Media”). The undersigned further grants the Company the perpetual, worldwide, royalty-free right to use, reproduce, publish, display, distribute, and otherwise exploit such Media, in whole or in part, in any format or medium now known or hereafter developed, including but not limited to social media platforms, the Company’s website, and promotional and marketing materials, without further notice, acknowledgment, or compensation.

2. Creative Use and Alteration

   The undersigned grants the Company the right to edit, modify, crop, retouch, or otherwise alter the Media, provided that such alterations are not made in a manner that is defamatory, obscene, or materially misleading so as to harm the reputation of the undersigned, their spouse, or their minor child(ren).

3. Consent to Use for Promotional Purposes

   The undersigned expressly consents to the use of the Media by the Company for advertising, publicity, marketing, and promotional purposes, in any and all media formats, without limitation.

4. Release of Liability and Waiver of Claims

   To the fullest extent permitted by law, the undersigned, on behalf of themselves, their spouse, their minor child(ren), and their respective heirs, assigns, personal representatives, and next of kin, hereby releases, waives, and discharges the Company and its directors, officers, agents, employees, contractors, representatives, successors, and assigns from any and all claims, demands, or causes of action arising out of or related to the use of the Media, including but not limited to claims for invasion of privacy, right of publicity, defamation, or any other personal or proprietary rights.

5. Representation of Authority and Age

   The undersigned represents and warrants that they have the legal authority to execute this Agreement on behalf of themselves, their spouse, and/or their minor child(ren), as applicable, and that at least one signatory is eighteen (18) years of age or older.

6. Acknowledgment and Voluntary Execution

   The undersigned acknowledges that they have carefully read this Agreement, fully understand its terms, and understand that they are waiving substantial legal rights. The undersigned further acknowledges that this Agreement is executed freely and voluntarily, without any inducement, assurance, or guarantee.
